A Notice of Violation issued by the Department of Consumer and Regulatory Affairs generally orders abatement of a violation within a specific period of time.  If you have received a Notice of Violation, you may appeal it to the Office of Administrative Hearings within a specified period of time.  Although the form provided here is not required, you may use it to appeal the Notice.  You should include a copy of the Notice of Violation when you file the appeal request.

This form should not be used in connection with Notices of Violation issued by any other agency.
