If you wish to appeal a decision of the Department of Employment Services (“DOES”) concerning Unemployment Compensation benefits, please use the Unemployment Insurance Appeal Form to request an appeal. You can find the Appeal Form below. Your appeal must be either postmarked by the U.S. Postal Service or received by the Office of Administrative Hearings within fifteen (15) days of the date DOES mailed the Claims Examiner’s Determination (CED) to you. For OAH to fully process your appeal request, you must attach the DOES Claims Examiner’s Determination (CED) to your appeal request.
