You may use a form attached below to request a copy of an audio recording of a hearing held by the Office of Administrative Hearings (OAH). OAH uses digital recording programs to record all hearings. Recordings can be delivered by mail or email. If you request email delivery, you must have a PC or laptop with internet access to receive and play the hearing. If you request mail delivery, OAH will send you the recording on a CD or DVD, so you must have a device capable of playing the CD or DVD to hear the recording. Further instructions on how to play the audio recording will be included with the delivery. Please allow five business days for OAH to process your request.

Puede usar este formulario para solicitar una copia de una grabación de audio de una audiencia celebrada por la Oficina de Audiencias Administrativas (OAH).
